ADT + RT reduces cancer-specific and all-cause mortality in older locally-advanced prostate cancer
In a SEER‑Medicare emulation (n=5,494; 3,378 received ADT+RT), ADT+RT was associated with lower cancer-specific mortality (HR 0.61) and lower all-cause mortality (HR 0.84). Results were consistent across frailty, stage, Gleason score, and PSA after sIPW adjustment with median follow-up 64 months. Higher claims-based frailty reduced the likelihood of receiving ADT+RT (adjusted OR 0.81 per unit CFI), despite preserved cancer-specific benefit.
